• Physician associate/assistant license in FL, GA, NV, NH, OK, TN, or TX?
• Respiratory therapy license in AL, FL, GA, MS, NH, or OK?
• Emergency medical services license in TN?
• Pharmacist license in FL?

Your license number is required for us to report credits to CE Broker. The information must match your licensure board first/last name and license number (in exact format for that board), otherwise, we will not be able to report your credits through CE Broker.

You will be prompted to provide or verify license information in your online evaluation, which we will use to report your credits.
